Use this form to apply for a loan. Print out and return with payroll and tax forms as explained on page 1. Fill out and mail in or drop off (we will need your original signature). Do not use this application for Mortgage, Home Equity, Visa, or Home Equity Lines of Credit Loans.

Visa Platinum Credit Card Application
Use this form to apply for a Visa credit card, or to request a new Visa rate. Include payroll and tax forms as necessary. Complete the application, then mail in or drop off (we will need your original signature). (Do not use to order a Visa debit card.)

Home Equity Application
Use this form to apply for a home equity loan in Pennsylvania only. Print out, complete and sign the documents. Mail in or drop off at one of our locations (we will need your original signature). Do not use this application for Mortgages or Visa.

Skip-A-Pay
Complete this form to skip a Penn State Federal loan payment. Upon completion, mail in or drop off. For Skip-A-Pay questions, contact our Loan Department.

W-8BEN Form
Need a W-8BEN to submit with your membership application? If you are not a U.S. citizen, you will need to add this to your membership application. Complete this from and return it to Penn State Federal.

Account Change Form

Fill out this form to order a new or replacement VISA debit/ATM card. Fax it in, mail it in or drop it off.

Penny On-Line Application
Please complete the application. You may then scan or take a picture of the application and email it to backoffice@pennstatefederal.com, fax it to 814-865-7713, or mail or drop off to Penn State Federal Credit Union, Attn: Back Office, 123 Amberleigh Lane, Bellefonte, PA 16823. Once we receive your completed application, a Credit Union Specialist will contact you by phone or email to finish the activation. After your online access is set up, you may then download our App in the App Store or Google Play Store to use mobile banking.

Wire Transfer Request

CD Renewal Letter
